Portal Patient Portal is a technological tool to securely store your medical imaging information and give your healthcare professionals access to your reports and images. It is not intended to provide you with medical advice. Your referring physician is the most equipped to interpret those results in the context of your personal medical history. Please discuss any results or questions that you may have with your physicians
Consent for Disclosure of Personal Health Information:
Your referring physician will receive your report based on the tacit consent that you have provided him/her when he/she sighed your referral form.
Any other entity requesting a copy of your medical records will be required to send us a release of information form signed by you.
Requesting Medical Records:
If you are requesting medical records for someone else, and if the person signing is not the patient, please provide us with documentation of your authority to obtain this information.
During The Exam:
Our technologists are not authorized by law to discuss any portion of your exam with you. Please discuss any questions or results with your referring physicians
